38 Year-Old Male from Indianapolis, Indiana with an alleged onset date of disability of 1/30/2020 and a date last insured of 12/31/2025.

The claimant has chronic pain, degenerative disc disease of lumbar spine mild-moderate, Phenylketonuria, anxiety disorder, depression, vision impairment with uncorrected vision of 20/200 bilateral and corrected vision 20/80 bilaterally.

The claimant was evaluated by a consultative ophthalmologist who found the claimant to have uncorrected vision at 20/200 (legal blindness), but corrected vision 20/80 which is associated with blurred vision at a distance and even close up.
